Thank you for taking the time to complete this survey and for sharing your thoughts on working at Acme. This survey is designed to obtain your opinions and perceptions of your job and work environment. The results will help Acme to identify what we are doing well, major concerns, and opportunities for improvement.

Your responses will remain anonymous.  No one at Acme will see your individual results, whether you complete either a paper or an online survey.  Schroeder Research Inc., an independent research firm, will process the survey results, including comments to the open-ended question, and prepare a report for the Senior Leadership Team. The overall results of the survey will be presented to all staff and to the Acme Board of Directors.
